To request family and medial leave:

* fill out a request for Family/Medical Leave form available in HR

*ask the appropriate physician to complete the Certification of Health Care Provider form, if necessary: and

* leave the two forms with HR

A Response to Employee Request of FMLA will be mailed to the employee by NewPage within a reasonable amount of time, after the forms are received. Please direct requests for this benefit to Human Resources
