 | Retirements must always
be the 1st of the month |
 | Employees must have
enough vacation/bh/ppd's etc. to get them from their last day at work until
their retirement |
 | Employees must contact
the Service Center no more than 90 days before the retirement and no less than
45 days before retirement in order for the retirement check to be timely -
phone number is 1-866-310-6955, option 2, option 2 again will get them
directly into the retirement. |
 | Once the employee has
contacted the Service Center they need to contact someone in HR so that we can
enter the retirement date in our systems.

 | Retiree insurance
information is sent out approximately 10-14 days after retirement date.
Retirees have 31 days to make an election in a retiree plan, so it is
important they contact the Service Center if they don't receive the
information within a couple of weeks. |
 | Dental is offered for
only 18 months (through COBRA). Then coverage is done.
|
 | Vision is offered each
enrollment year. |
 | Employees have a
$1,000.00 life insurance policy when they retire.
|
 | Employees can convert
their current life insurance policy from group to individual when they retire
- again limited time to convert, this needs to be done at retirement time.
